🎓 Career in China CN
(To be a consultant)

Step 1: MBBS degree- Complete your primary medical qualification from your home country. It must be from a WHO/MOE-recognised institution.

Step 2: Chinese language (Mandarin / HSK)- This is the biggest hurdle for foreign graduates. Non-native speakers must prove proficiency in Mandarin by passing HSK, and achieving advanced level requires on average 2–3 years of dedicated study. You need at minimum HSK Level 4 for internship, and HSK Level 5–6 to sit the NMLE and practice clinically.

Step 3: Degree verification & visa- Ensure your degree is from a WHO-listed and MOE-recognised university. Obtain the appropriate visa (work or study) to enter China. To actually work in a Chinese hospital after licensing, you will need a valid Work Permit — a separate process from licensing.

Step 4: Clinical internship (1 year)- Complete a 1-year internship in a Chinese-accredited hospital. Upon graduation, the graduate must work as a resident physician for a period to be eligible to take the NMLE for physician certification.

Step 5: NMLE (National Medical Licensing Examination)- The NMLE is divided into two parts: the written examination and the clinical skills examination, with a passing score of 60% for each part. The entire exam is conducted in Mandarin. Allocate 6–12 months of preparation.

Step 6: Physician license & work permit- Register your license with the local Health Bureau. Then apply for a foreign work permit sponsored by your employing hospital.

Step 7: Guipei residency training (3 years)- 'Guipei' is standardised training for residential physicians, consisting of a minimum of 3 years of rotating across secondary care departments. Being hired at a government-run hospital virtually requires this certificate.

Step 8: Specialist fellowship training (2–4 years)- After Guipei, enter sub-specialty training in your chosen field. After completing residency training, doctors must pass board certification exams relevant to their specialty.

Final step: Specialist consultant- After passing board exams you become an attending physician, and with further experience, an associate chief physician — the equivalent of a consultant. Specialist consultant level (10+ years experience) earns RMB 40,000–100,000+/month in major cities.

**Key notes**
-The NMLE is entirely in Mandarin. Without strong Chinese language skills, this pathway is not viable. Invest heavily in Mandarin from day one.

🎓 Career in China CN
(To be a surgeon)

Step 1: MBBS degree- Hold your primary medical qualification (MBBS or equivalent) from your home country, or complete a 5+1 year clinical medicine degree at a recognized Chinese university (the 6th year is the mandatory internship).

Step 2: Chinese language (HSK)- Learn Mandarin and pass the HSK (Hanyu Shuiping Kaoshi) exam. Passing HSK Level 6 is required to prove language proficiency for medical practice in China. This is critical because preparing for the NMLE requires deep Chinese language fluency. Reaching HSK 6 typically takes 2–3 years of dedicated study.

Step 3: 1-year internship in China- After obtaining a recognized Chinese medical degree, foreigners must complete a one-year professional training (internship) under the guidance of practicing doctors in medical institutions in China before applying for the Medical Licensing Examination.

Step 4: NMLE (National Medical Licensing Examination)- To practice medicine in China, medical graduates must pass the National Medical Licensing Examination (NMLE). The NMLE is divided into two parts: the written examination and the clinical skills examination, with a passing score of 60% for each part.

Step 5: Physician's Practicing License & Work Permit- After passing the NMLE, register with the local Health Administrative Department to obtain your Physician's Practicing License. To actually work in a Chinese hospital, you will also need a valid Work Permit — a separate process requiring a job offer from a sponsoring hospital.

Step 6: Residency / Guipei (3 years)- Guipei is a mandatory 3-year residency after the NMLE and is required by most hospitals before independent practice. Residency programs in China can last 3 to 5 years depending on the specialty, with competitive entry for the most sought-after fields like surgery. During this stage you select your surgical specialty.

Step 7: Surgical specialist training & fellowship (2–3 years)- After residency, complete a surgical fellowship/super-specialization program. Fellowship programs offer opportunities for super-specialization, requiring an additional 1 to 3 years of study post-residency.

Final step: Specialist registration & independent practice- Register as a specialist surgeon with the relevant Chinese health authority. China has its own physician career ladder: junior doctor → doctor in charge → assistant chief doctor → chief doctor — a stepwise system in which graduates progress within a period of about 15 years.

🎓 Career in China CN
(To be a researcher)

Step 1: MBBS degree- Complete your primary medical qualification from your home country.

Step 2: Language test- 'Chinese (HSK)': Required if your program is taught in Chinese. HSK level 4–5 is typically needed for postgraduate study.
'English (IELTS/TOEFL)': If joining an English-medium program — IELTS 6.0–6.5 or TOEFL 80+ is usually required.
Many top universities (Peking, Fudan, Zhejiang) offer English-taught programs, so HSK is not always mandatory.

Step 3: Apply for admission & scholarship- Apply to Chinese universities or research institutes. Key funding options: Chinese Government Scholarship (CSC), university-direct scholarships, or MOFCOM scholarship.

Step 4: Master's degree (2–3 years)- Complete an MSc or MPhil in a biomedical or clinical research field at a Chinese university.

Step 5: PhD (3–5 years)- Pursue a research doctorate at a Chinese university or institute such as the Chinese Academy of Sciences (CAS). Focus on publishing in indexed journals.

Step 6: Postdoctoral fellowship (2–3 years)- Strongly recommended before applying for independent faculty positions. Boosts your publication record and research network.

Step 7: Apply for a research position- Apply to university faculties, CAS institutes, or hospital-based research centres. Having SCI-indexed publications greatly improves your chances.

Step 8: Work visa & residence permit- Your institution sponsors a work permit and R or Z visa, which is converted to a residence permit upon arrival.

Final step: Medical researcher in China- Publish research, apply for NSFC (National Natural Science Foundation of China) grants, and build your research group or lab.
